Birth Proof: School Leaving / SSC / Transfer Certificate / Birth Certificate (Mandatory for DOB after 26.01.1989)
Photo ID: PAN Card / Election Card / Driving License
Address Proof:

Bank Statement (1 year), Office & Appointment Letter, IT Returns (Last 3 years)
Ration Card, Election Card, Society Letter, MSEB / BSNL Bill (1 yr, 6 months, latest)
Home Tax Receipts (Last 3 years)

Educational Documents: SSC & Degree Certificates + Mark Sheets

Important Notes:

NOC Requirement: If you are employed in a Government/Semi-Govt. organization, nationalized bank, aided school/college/university, you must submit a No Objection Certificate (NOC) from your department head.
Police Verification: Local police conduct the verification after receiving details from the passport office. You may follow up on Tuesday, Thursday, or Saturday at your local station.
Urgent Cases: For urgent requests, applicants must visit the passport officer with the file number (receipt). Final approval lies with the officer.
